USB32HD4 started crashing with 4 monitors, worked for years

My USB32HD4 quad adapter started malfunctioning. need assistance with a repeatable failure of my StarTech quad-HDMI USB adapter on a Dell Precision 7560 running Windows 11 25H2, build 26200.9457.

Three external monitors work. Connecting a fourth causes all external displays to go black. Disconnecting the fourth generally restores the other three without further interaction.

The failure follows the number of connected monitors, regardless of which physical HDMI port or monitor is connected last. Every output channel has worked independently, and this setup previously operated with all four external monitors.

Completed troubleshooting:

  • Tested different monitor combinations, connection sequences, and USB hubs/docks.
  • Checked USB and display-device enumeration extensively.
  • Removed and reinstalled the graphics driver twice, with the same result.
  • Restarted Windows and reset the USB connection.
  • Corrected duplicate-display settings to use an extended desktop.
  • Tested at 1920 × 1080 throughout, including reducing the three working external monitors from 60 Hz to 50 Hz. Adding the fourth still caused the blackout.

Current signed drivers:

  • Trigger 6 External Graphics: USB\VID_0711&PID_5621, trgidd64.inf, version 1.9.2501.1223, dated December 23, 2025. Both graphics devices report Started.
  • T6 USB Station: USB\VID_0711&PID_5601, t6sta.inf, version 1.0.2501.1021, dated October 21, 2025. Both USB station devices report OK.

An ARM64 package was briefly added during troubleshooting but did not bind to the devices. The currently installed graphics driver is confirmed x64.

Windows recorded LiveKernelEvent 0x1B8, parameter 1 = 1. WinDbg identifies this as a hotkey-triggered black-screen diagnostic capture; its stack does not establish the faulty component. The dump is available as WATCHDOG4400-20260916-1156.dmp. Reproducing the blackout with the DriverFrameworks-UserMode Operational log enabled produced no events.

Please escalate this to someone who can assess four-output initialization failures. Can you confirm whether these graphics and USB station driver versions are a supported combination, identify any known Windows 11 25H2 issues, and specify the diagnostic capture needed to distinguish a driver problem from an adapter hardware or firmware problem?

I would appreciate targeted next steps beyond repeating the connection tests and driver reinstalls already completed.

A couple of quick steps in the meantime. Try a graphics reset: With all 4 monitors connected, Press: Windows Key + Ctrl + Shift + B.

If you have access to a second Windows computer, consider testing the USB32HD2 on that computer to see if the issue persist there. This can help us quickly narrow down if the issue is related to the hardware.
